Discharge
Hospital stay should be as short as possible and you should be informed of discharge within one day of admission.
Before you leave the hospital you are entitled to receive:
Information on admission, treatment and study results.
Instructions for continued monitoring and where to located results that are not ready before discharge.
Information on warning signs to watch for after returning home and where to seek help if problems arise after discharge.
Overview of the medicines you should take and the duration of their use. Prescriptions are sent to the prescription portal.
If necessary, discharge may include support at home or admittance elsewhere.
Who discharges you?
The discharge team at Landspítali consists of nurses, social workers and doctors. The team advises patients, family members and staff if:
services from different sources such as social services, home care and health assessment committees need to be coordinated.
special measures are necessary, such as care needs or placement assessment.
