Working conditions at Landspítali are based on Icelandic collective agreements, which ensure transparent terms and equal treatment.

The standard work week is typically 36 hours for shift-based roles within the hospital, while some roles may vary depending on the position and agreement. Salaries are determined by collective bargaining agreements and are based on education, experience and role. Additional payments are made for shift work and overtime.

Employees accrue paid vacation at a minimum of 24 working days per year (equivalent to 2 days per month). In some roles, particularly shift work, additional winter leave may apply. Notice periods are usually between 1 and 3 months, depending on employment terms and length of service.

Income tax in Iceland is progressive (approximately 31–46%), and all employees receive a personal tax credit which lowers the effective tax rate. Employees contribute around 4% to a pension fund, with additional employer contributions. Most employees are also members of a labour union.

Individuals who are legally resident in Iceland are generally covered by the Icelandic social insurance system after six months of residence.

For citizens of EEA/EU countries, earlier access to healthcare coverage may apply under international agreements. Until full coverage is established, individuals are responsible for healthcare costs but may seek reimbursement through their insurance provider in their home country or private insurance.

All employees are covered by occupational injury insurance while at work.

Icelandic is the primary working language at Landspítali. Internal systems, guidelines and most day-to-day communication are in Icelandic.

At the same time, English is widely spoken, and many teams use English as a support language, especially during the initial period of employment. However, learning Icelandic is strongly encouraged, as it is important for both professional and social integration.
