Priority 2: Improving drug safety in multiple-drug therapy

Improved drug therapy process

The goal is to improve the drug treatment process through the use of evidence-based tools to guide the prescription and review of drugs, to strengthen the involvement of clinical pharmacists and to improve the health literacy of patients in multidrug therapy.

Pilot projects have been started in healthcare where emphasis is placed on clinical pharmacists at Landspítala sharing their expertise outside the hospital.

Strengthening Clinical Pharmacy Services

A working group from Landspítali (National University Hospital of Iceland) and primary healthcare centers is developing a nationwide strategy for clinical pharmacy services.

Iceland currently has too few clinical pharmacists to provide comprehensive support to multidisciplinary clinical teams in improving medication safety in polypharmacy. The strategy focuses on prioritizing the limited workforce of clinical pharmacists to maximize their impact.

A pilot project is currently underway at several primary healthcare centers, where clinical pharmacists assist general practitioners in reviewing prescriptions and tapering medications.
