Priority 1: Improved transfer of medication

The goal is to reduce medication errors during transitions of care and treatment in Iceland by:

  1. Coordinate medication therapy – All patients receive timely and standardized medication reconciliation when transferring between healthcare providers.

  2. Enhancing patient knowledge of their prescribed medications – Patients and caregivers receive clear, up-to-date information about their medications.

  3. Ensuring comprehensive transfer and discharge records – Clear, timely, and detailed written information on a patient’s medication regimen is provided between healthcare providers.

  4. Supporting the implementation of a centralized medication record – Physicians are responsible for prescribing and updating a centralized medication record, improving access for healthcare professionals and patients to clear, updated medication information through a unified system.

Projects have been launched at Landspítali (National University Hospital of Iceland) and a primary healthcare center in the Reykjavík area.
