Referrals to Landspítali can be sent in two ways:
Referral in Heilsugátt (for Landspítali staff and external users and institutions with Heilsugátt access).
"Referral" form in the Saga system (for external institutions).

It is best to send a referral through Heilsugátt (Health Portal): Tilvísanir (Referrals) / Göngudeild Gigt (Rheumatology Outpatient Clinic)
The Rheumatology Outpatient Clinic accepts referrals for:
GCA Fast-Track Clinic (30 min): Rapid workup if Giant Cell Arteritis (GCA) is suspected. Please call the consulting rheumatologist and send the referral. Rapid GCA workup includes a blood test, interview, and examination, followed by ultrasound of the arteries within 2-3 days. It is important to call a rheumatologist, as treatment and examinations must be tailored to each individual.
Joint Screening (15 min): We offer joint screening where a referral can be sent for the examination and ultrasound of joints in a patient with joint pain where it is unclear whether joint inflammation is present or not. An interview is not included, only examination and ultrasound.
Diagnosis/confirmation of inflammatory joint disease (RA, PsA, ReA) and initiation of treatment. Treatment of PMR if immunosuppressive drugs are needed, i.e., if it has not been possible to taper off steroids within 1 year, the patient cannot get below 10 mg of prednisolone per day, or the patient tolerates steroid treatment poorly due to, e.g., diabetes.
Complex treatment of inflammatory joint diseases (RA, SpA).
Crystalline arthritis that does not respond to conventional treatment (colchicine/allopurinol).
Diagnosis and monitoring of severe multi-system diseases, e.g.: Vasculitis (ANCA-positive vasculitis, Behcet's), Connective tissue diseases: Systemic Lupus Erythematosus (SLE), Systemic Sclerosis (systemic sclerosis), Polymyositis/dermatomyositis.
The Rheumatology Outpatient Clinic does not accept referrals for the treatment of fibromyalgia, Ehlers-Danlos, or osteoarthritis.
Information in Referrals to Landspítali Rheumatology Outpatient Clinic:
All referrals should include information about medication, history, examination, and family history of rheumatic disease. Also, general blood tests (no older than 1 month) should be taken: Complete Blood Count (Blóðhagur), CRP, ESR (sökk), Creatinine, ALAT. In addition, we request that referrals contain the following information:
If Connective Tissue Disease is Suspected (SLE, SSc, Sjögrens, APS)
If the patient has the following symptoms, please describe further:
Alopecia, significant hair loss
Dry eyes, dry mouth
Sun sensitivity (photoallergy)
Eye inflammation (uveitis)
Skin rash (especially butterfly rash, telangiectasias, skin hardening)
Joint pain, joint inflammation, morning stiffness
Blood clots, miscarriages
Raynaud's
Chest pain due to pleural irritation, shortness of breath
Difficulty swallowing
Tests: ANA and ENA blood test and urine test
If Inflammatory Joint Disease is Suspected (RA, PsA, ReA)
If the patient has the following symptoms, please describe further:

Skin rash – psoriasis
Morning stiffness
Tests: RF and/or anti-CCP and uric acid (blood test)
If PMR or GCA is Suspected

Can lift arms
Response to steroid treatment
Symptoms of GCA are present, i.e., new headache over temples, jaw claudication, scalp tenderness, visual disturbances
If Spondyloarthritis (ankylosing spondylitis) is Suspected
If the patient has the following symptoms, please describe further:
Joint inflammation/ joint pain and in which joints
Back pain, location
Morning stiffness, night pain
Do pains improve/worsen with rest or movement, anti-inflammatory drugs
Skin rash – psoriasis
Tests: A CT scan or MRI of the spine and SI joints is recommended.
Referrals
Acute Care: Referrals for acute care are accepted from professionals within Landspítali, especially from the emergency department and the Landspítali Emergency Department.
Treatment Line: Professionals inside and outside of Landspítali are welcome to refer patients if PTSD is suspected to be the main issue, in conjunction with other mental health disorders.
Referrals should be sent to the intake team of the Mood Disorder Treatment Unit, and it is essential to include a rationale describing the current active PTSD symptoms and the trauma that caused them.
We encourage those who seek advice or need their issue assessed to consult their general practitioner, who will evaluate the case and send a referral to the Eating Disorder Team at Landspítali if necessary.

To apply for the services of the community mental health team, a request must be submitted to the intake team for psychosis treatment.
Requests are only accepted from professionals.
Once a request has been received by the team, it is reviewed within two weeks. At that point, it is assessed whether the individual is considered to be in the target group of the community mental health team. If so, the person is called in for a preliminary interview as soon as possible.
The Depression and Anxiety Team (ÞOK) is a multidisciplinary outpatient team that provides diagnosis and treatment for severe anxiety and depressive disorders when milder treatments have not been effective.

The DBT Team (DAM teymi) is a multidisciplinary team that provides Dialectical Behaviour Therapy (DBT) for individuals with severe emotional difficulties.

The Bipolar Disorder Team is a multidisciplinary outpatient team that provides specialized treatment for individuals with bipolar disorder.
Professionals send a referral including the following information (in icelandic):
The outpatient department for mental health and addiction accepts referrals from psychiatric inpatient units, emergency departments of Landspítali, other healthcare institutions, child protection services, and municipal social services.
Laugarás is for young people (18 - 35 years old) with early-stage psychotic disorders
Any professional can send a request to Laugarás and the Psychosis Team.

Referrals to the intake team of the Mood Disorders Treatment Unit must be submitted electronically via Heilsugátt.
If that is not possible, referrals can be mailed to:

The intake team meets once a week to review referrals.
If a referral is accepted, it is placed on a waiting list for the appropriate specialized outpatient team. The referring clinician receives a letter confirming whether the referral has been accepted.
If accepted, the patient receives a letter with information about the estimated waiting time and will be contacted to schedule an appointment when their turn comes. If the referral is declined, the patient receives a letter stating this.

Professionals in a child's local area refer to the Child and Adolescent Psychiatry Department
Diagnosis and/or treatment teams, specialist physicians, primary care doctors, psychologists, and other specialists, as well as staff from social services, school services, and Child Protective Services, refer to the service. These professionals assess the problem in the context of the child's family, environment, and school, and provide appropriate support and treatment options.
Before a child is referred to the Child and Adolescent Psychiatry Department
If there is a shared assessment of the need for more specialized services, it is the responsibility of the specialist who performed the initial assessments and preliminary diagnosis to send a referral to BUGL.
Referral
The following must be sent to the BUGL Admissions Team:
A completed (.pdf in Icelandic) form. The referrer fills it out, and a local doctor or another doctor who has attended to the child must sign the referral to ensure continuity and follow-up in the child's treatment. The form can be filled out digitally and then printed.
A completed (.pdf) form. It is necessary for parents to sign the Authorization to Obtain and Provide Information. If there are two legal guardians, both must agree to the referral, but one signature is sufficient.
A preliminary diagnosis.
Information about the child's issue.
Results from developmental assessments, such as WISC-IV or WIPSI tests.
Results and numerical data, profiles of other assessment lists and screening or diagnostic tools.
Other diagnoses, results, and data that can shed light on the case.
All professionals can contact the team.
Within Landspítali, requests are sent to the hospice counseling team as a "Request for advice" in Saga.
Outside Landspitali, a “referral” is sent to Heilsugátt
It is important that a request state the reason for seeking counsel. The team’s employees evaluate the request within 24 hours.

It is important to fill in the request well. The laboratory staff will provide information as needed.
Samples
Most DNA-based molecular genetic studies require 4 ml EDTA anticoagulant blood.
It is important to roll the bottle several times
The sample is stable at room temperature for 3 days
If you have any doubts about the sample tube type, ask the laboratory for advice.
Samples must be sent to a laboratory in building K near Hringbraut for sampling.
References should be received
either in:
Saga, on a form under references between institutions. Mark the recipient of the geriatrics outpatient clinic.
Heilsugattl, electronically, if available to the physician in question.
It must be stated:
reason for the reference
health history
patient's medication
name and contact number of the doctor who sends the request.
Bear in mind:
The referring physician must have assessed as far as possible whether there is a temporary condition that could be caused by stress, trauma or physical illness.
It is preferable that the referring physician has assessed cognitive abilities with an interview with the patient and family member where appropriate and with simple tests such as MMSE and a clock test.
The Landspítali Pain Management Center provides care for individuals experiencing severe or persistent pain that has not responded to conventional treatment or requires specialized intervention. Demand for the service is high, so it is important that the patient has undergone appropriate evaluation and received recommended treatments before a referral is made. Comprehensive information must accompany all referrals to ensure proper prioritization and triage.
Only electronic referrals sent through the Saga system and addressed to the Landspítali Pain Management Center are accepted. Please include the following information before sending a referral:
Personal information, employment status, and social circumstances
Description of the problem: pain location, onset and circumstances, characteristics of the pain, and its impact on daily life. Include details of previous treatments.
Results of imaging and other diagnostic tests performed to investigate the pain
Request for consultation for inpatients:
Requests for consultation must be sent through Saga and addressed to the Pain Management Center (from the requesting ward). Requests are answered as soon as possible, but same-day consultation cannot be guaranteed. If the case is urgent, it is recommended to call and inform the Pain Management Center of the request.

Please note: For urgent advice regarding acute pain, contact the on-call anesthesiologist.
After initial assessment, the physician refers the patient to the Landspítali Sarcoma Team through Heilsugátt.
The following tests must be completed before referral:
MRI of soft tissues
X-ray and CT scan of bones
Professionals who do not have access to Saga or Heilsugátt can send certain referrals to Landspítali through Signet Transfer.
Signet Transfer is a software solution that allows authorized users to send sensitive information to other parties or institutions. All documents are encrypted end-to-end according to standards, and receipt of documents is confirmed using electronic ID. This ensures that no unauthorized party can access the data transfer.
