Skills and health assessment

Skills and health assessment is a professional, individualised assessment of the needs of individuals for permanent residence in nursing and residential homes.

This is a standard assessment, along with

  • defined information from health and social services

  • Regional Office for the Disabled

  • medical certificate from the person's general practitioner, as applicable

Skills and health assessments for temporary rest in nursing homes are also conducted. Usually, it is assumed that the request for a skills and health assessment or a rest period will not exceed 4-6 weeks from the date the committee receives the decision and is notified in writing.

The application must be sent to the Skills and Health Assessment Committee in the health region where the person concerned is domiciled.
