Kindly note that if a patient ID/Icelandic social security number or a time period is missing from the form, it will not be possible to process your request. For those without an Icelandic social security: Your patient ID is a combination of 10 letters and numbers, starting with your date of birth. It is on your medical bracelet and/or any papers (receipts, bills) you might have received while in the hospital.
