A parent or guardian of a child has a right to see the child's medical record.

Children 16 years and older

• Children who have reached the age of 16 are independent service users of the healthcare system and can seek medical advice or other healthcare professionals without the consent or knowledge of a guardian.

• A request from parents for a copy of medical records for children aged 16 to 18 must be accompanied by a signed authorization from the child themselves.
