Disability assessment

The first application must be accompanied by:

• a medical certificate for the application for disability assessment,

• a questionnaire on impairment, which is completed on My pages,

• confirmation from the pension fund that payments have been applied for or that there is no right to receive payments;

• ,

• information on the use of a tax card,

If you were in rehab, you must also hand in:

• a statement by the doctor or rehabilitation provider that rehabilitation is complete;

or

• service termination report from e.g. Virk.
