Provides assessment, treatment, support, counseling, and follow-up for individuals aged 67 and older who are living with chronic health conditions and declining functional ability and for their caregivers.
Services and Specialized Clinics
General Clinic
Evaluation of treatment and rehabilitation options for chronic conditions and reduced function. Follow-up is provided when coordination of complex care services is needed.
Falls and Fracture Prevention Clinic
Physician referrals are accepted for patients experiencing repeated falls. The clinic identifies underlying causes and provides prevention strategies.
Memory Clinic
Diagnosis of dementia and memory-related disorders. Common symptoms include:
Memory loss
Disorientation
Apraxia (difficulty with motor planning)
Aphasia (language difficulties)
Psychiatric symptoms (e.g., delusions, depression, anxiety, sundowning)
Nursing Clinic
Assessment of physical and mental health, social circumstances, and care needs. A personalized care plan is created and followed up as needed.
Geriatric Assessment Clinic
For elderly patients with complex and multiple health conditions. Often referred after repeated emergency visits when additional nursing support is required.
A referral from a physician is required to access outpatient services at Landakot.
