Risk Factor Assessment

During hospitalization, the focus is on:
Addressing risk factors, such as reviewing medication lists, managing orthostatic hypotension, diagnosing and treating delirium, and providing nutritional counseling.
Teaching patients techniques to enhance stability and safety in daily activities.
Eliminating environmental fall hazards and providing appropriate assistive devices.
Before Discharge
Studies have shown that balance training can help prevent falls, and exercise programs for fall prevention in older adults can reduce injuries and decrease fall-related hospitalizations.Before discharge, in consultation with the patient and their family:
The need for a home safety assessment is evaluated.
Appropriate assistive devices are provided, along with training on their use.
Balance training and other rehabilitation programs are arranged.
While it is unrealistic to prevent all falls, international research shows that targeted interventions can significantly reduce fall incidents, thereby minimizing patient suffering and healthcare costs.
