Information, forms, and applications for midwives
If a midwife wishes to operate under an agreement between Iceland Health and the Association of Home-Visiting Midwives regarding the services of independently practicing midwives, the first step is to notify Iceland Health. To do this, one should visit Iceland Health website – Iceland Health - Select the category Contracts – Procurement and enter in the subject 'Application for Service Agreement – Midwife.' The midwife shall then provide information on when she intends to commence work.
Before the application can be approved, all required accompanying documents must have been submitted, i.e., a copy of the professional license, a copy of the confirmation from the Directorate of Health on independent midwifery practice, and a copy of professional liability insurance from an insurance company, in addition to applying for patient insurance through Iceland Health Gateway. For the purpose of ensuring that the accompanying documents are submitted in the correct format and to the correct destination, they must be sent via Iceland Health Gateway. Once the application has been submitted, the midwife will receive Terms for access to Iceland Health information system for electronic signing.
Iceland Health does not cover bills for procedures performed prior to the signing of a service agreement.
It is the responsibility of the midwife to submit the application and the required documents to Iceland Health.
