Advice is also provided on choosing a nursing home and the resources available to individuals to continue living in their own home.

Generally, it is assumed that no more than four weeks will pass from the time the committee receives a request for a skill and health assessment until a conclusion is available. The committee's employee requests certificates from healthcare professionals involved in providing services to applicants. The conclusion of the evaluation is announced in writing as soon as it is available.

When a person moves to a nursing home at the Healthcare Institution of East Iceland, we ask the relatives to fill in a life biography format, called Life History. The questions help the employees get to know the person moving into the home.

The information is helpful in approaching the person in care according to their habits and values, to have meaningful communication and to support them to engage in activities, that are in their field of interest.

Employees can assist as needed when filling out the forms. Our biography format is from the Alzheimer's Association with slight modifications. It is best to fill in all aspects of the life history, but of course, you are free to leave out individual details.
