Case managers in services for children's prosperity

If there is a request from parents and/or the child for integration of services, and there is reason to believe that the child needs multi-level services at the second and/or third level for a long period of time, the municipality where the child is domiciled shall appoint a case manager for services in the interests of the child's prosperity. The municipality shall select a case manager who works in the social services of the municipality or in the areas where the child's needs are concerned. The case manager shall have knowledge of services in the interests of the children's prosperity.

The case manager for children's prosperity is:

  • An employee working in the social services of the municipality or where the needs of the child are concerned

  • The person who leads a support team at the second and/or third level of services in the interest of children's prosperity

  • Supporting families in integrating services at second and third level

  • A person who has the interests of the child in mind and is cooperating and consulting with parents and the child

The role of the case manager:

  • Providing advice and information on services for the prosperity of the child.

  • Assisting in ensuring access to assessment and/or analysis of the needs of the child.

  • Responsibility for drawing up a support plan and managing support teams.

  • Following up on the provision of services in accordance with the support program.

  • Providing counseling and information to those who are members of the support team for integration of child services.

Before the district commissioner begins to provide services under paragraph 1, the parents and/or the child must make a formal request for the integration of services. When that request has been made, the district commissioner may obtain information on the situation of the child from service providers, and those who provide general services in the interest of the child's prosperity.

Support teams and support program

  • The case manager shall establish a support team consisting of representatives of the service providers who provide services to the child. Those who provide general services in the interest of the child's prosperity may be invited to join the support team if necessary.

  • The support team must have the child's interests at heart and fulfill their roles in collaboration and consultation with parents and the child. Parents (and, where applicable, the child) are part of the support team, attend its meetings and participate in the preparation of a support plan.

  • A support team shall as soon as possible draw up a written support plan for the child, integrating services provided for the child's prosperity. The support plan shall, where appropriate, include:

  1. Assessment and/or analysis of the needs of the child.

  2. The objectives of services and the integration of their individual components at all levels of service.

  3. The role of each service provider and others as appropriate.

  4. How progress will be evaluated.

  5. Duration of the plan

  • The support team cooperates regularly on the implementation of the support plan during the period of the plan. It shall be re-evaluated and renewed as necessary. Before the child reaches 18 years of age, the support team shall draw up a plan which addresses the integration of services for the child after reaching adulthood.

If a support team ends its work without a support plan, the parents of the child and the case manager shall be informed of the conclusion of the team, and the justification for it. The parent can address a complaint about the decision of the support team to conclude its work without a support plan to the Quality and Supervisory Authority of Welfare.
