Rehabilitation and Treatment for Older Adults
The ward provides care and rehabilitation for older adults transferred from acute hospital departments. Hospital stays should not be longer than necessary, and discharge plans are usually communicated within two days of admission.
The goal is to improve independence and physical, mental, and social functioning
Patients are encouraged to actively participate in daily activities, stay on their feet as much as possible, eat in the dining area, and wear their own clothes.
The average length of stay is 4 to 6 weeks, but may be shorter or longer depending on the individual’s condition.
The aim is always to discharge patients home with appropriate support. Discharge planning and applications for necessary services begin upon admission.
If returning home is not possible due to health or functional decline, a functional and health assessment is carried out in collaboration with the patient and their family. This assessment also serves as an application for a nursing home. Once approved, the patient chooses a nursing home and is advised to select at least three options.
Transitional (Waiting) Care
If a patient cannot wait at home for a nursing home placement, they may move to a transitional care unit. Landspítali operates a waiting unit on L-3 at Landakot. Other available waiting units include: Vífilsstaðir, Móberg in Selfoss, Brákarhlíð in Borgarnes, and occasionally nursing homes in the capital area.
