Monitoring and Treatment

During follow-ups, patient well-being is assessed, physical examinations and measurements are performed, medication regimens are reviewed, and adjustments are made if necessary.
Education and support are provided as needed, along with an evaluation of required services (e.g., home nursing).
Once the patient’s condition stabilizes, further follow-ups are managed by cardiologists or primary care physicians.
