What do I need to submit to have my medical and rehabilitation allowance assessed?
First time
Before applying for an allowance for the first time, the following must be available:
a medical certificate and one of the three confirmations, as applicable:
due to recognised treatment,
waiting for treatment or rehabilitation to begin,
when ill health prevents treatment or rehabilitation from beginning.

Renewal application
If you need to apply to continue receiving an allowance, the following documents must be available before you can apply:
