Palliative Consultation Team
The Palliative Consultation Team provides specialized palliative care and guidance both within and outside Landspítali. The goal is to improve the well-being and quality of life of patients with chronic or life-threatening illnesses, such as cancer, heart, lung, neurological, and kidney diseases.
What Does Palliative Care Involve?
Improved Well-being and Quality of Life: Preventing and managing disease symptoms and treatment side effects.
Holistic Approach: Addressing the physical, emotional, psychological, and social impacts of illness.
Collaboration and Support: Prioritizing discussions about disease progression, treatment plans, and patient preferences.
Services Provided by the Palliative Consultation Team
Consultation and Support: Offering guidance to patients and families through outpatient visits and phone follow-ups.
Collaboration with Healthcare Professionals: Assisting with symptom management, communication, and discharge planning, including referrals for home care or admission to the Palliative Care Unit.
Education and Research: Engaging in training, research, and development in palliative care.
Palliative care often begins alongside other treatments and can support patients over the long term. The focus is on comprehensive care that helps both patients and families adapt to changing circumstances.
Referrals
All healthcare professionals can request assistance from the team.
Within Landspítali: Requests should be submitted to the Palliative Consultation Team as a "Consultation Request" in the Saga system.
Outside Landspítali: A referral should be sent through Heilsugátt.
The team reviews referrals within 24 hours.
