Services
Landspítali (GoS) provides treatment and support to people with severe mental illnesses, particularly psychotic disorders, aimed at improving health and enhancing quality of life.
The service is tailored to individual needs with active participation of service users, and emphasis is placed on good cooperation with family and other relatives.
The team provides both services and treatment at the Kleppur outpatient clinic, as well as being a strong community mental health service.
Treatment providers
The team consists of doctors, psychologists, nurses, social workers, a developmental therapist, a peer support specialist, employment advisors, and other specialized staff.
It is intended that everyone receives their own case manager who oversees the service user's treatment and serves as their main point of contact.
Collaboration with other institutions is carried out as needed, such as primary healthcare and social services.
The teams are also in very close contact with the inpatient wards of the psychiatric department as well as residential units.
Service focus
The recovery-oriented approach that guides the work is based on the idea that recovery is possible and individual. The focus is on working with each person's strengths while enhancing participation in daily life and responsibility for one's own recovery.
Objectives
To:
provide interdisciplinary and personalized services to people diagnosed with severe mental illnesses and their families.
break social isolation, activate support networks, and encourage service users to actively participate and take responsibility for their own recovery.
reduce the need for hospitalizations and shorten hospital stays by ensuring good support and follow-up.
strengthen people's self-image and their choices regarding their own lives and the roles that are important to them.
ensure that service users receive the best possible treatment.
facilitate people's access and connect them to the appropriate services available in the community.
engage in appropriate collaboration, provide advice and support to others working in the field.
Services and treatment options:
The treatment options available include, among others:
cognitive rehabilitation (CR),
cognitive behavioral therapy (CBT) for substance abuse
CBT for psychotic symptoms.
Career counseling, peer support, regular support sessions and education for service users and family members.
The teams are also in good cooperation with the Recovery Center where a variety of activities are offered.
Case Management
In GoS, most service users have a case manager who oversees the treatment and serves as a liaison within the teams and also with other institutions. The focus of case manager treatment is flexible and depends on the circumstances and stage of illness of the service user at any given time.
Long-term goals of case managers are to promote recovery and prevent recurrent episodes of illness.
The responsibility of case managers is to have an overview of the service user's case and put together a treatment plan to meet their needs.
The role of case managers includes supporting service users in dealing with the consequences of psychosis, managing family support, and promoting recovery and increased participation in society again.
Within the teams, there is a special follow-up for individuals on Clozapine medication treatment. This follow-up is handled by nurses, as well as psychiatrists.
Case Manager
Manages the treatment process from the first visit to discharge.
Plans services in collaboration with the client.
Ensures continuity in treatment and connection to other services.
Service is personalized and regularly evaluated by the team and case manager.
Referral to the Psychosis and Community Mental Health Team (GoS)
Referrals are only accepted from healthcare professionals. Requests are sent to the psychosis treatment intake team through Heilsugátt (or Saga).
