Rehabilitation Unit for Older Adults
The Geriatric Discharge Unit provides rehabilitation for elderly individuals with the goal of discharging them to their own homes within 2 to 4 weeks of admission.
Goals of Treatment and Rehabilitation
The focus is on improving mobility and promoting independence in activities of daily living. Emphasis of care include:
activity, participation, and self-care during meals and other daily routines.
medical treatment and nursing care.
social work services.
physiotherapy and occupational therapy.
Comprehensive assessment including evaluation of medical conditions, age-related changes, social circumstances, medication management, and both physical and cognitive function. Care is provided by an interdisciplinary team.
Patients are encouraged to take an active role in their rehabilitation. They are responsible for their own training, including daily walks and completing prescribed exercises.
We recommend bringing personal clothing and enough changes of clothes, as patients are expected to wear their own clothes during the day.
