Services
Requests for home nursing must be submitted in writing by staff in healthcare or social services.
Health centers and Landspítali use the home nursing request form in the Saga-system and submit it electronically.
It is important to specify the reason for the request, the applicant’s medical diagnoses, and any health issues. Nursing and/or medical letters are requested to accompany the application.
If the request can be accommodated a nurse will visit, assess the nursing needs and plan the service in cooperation with the individual, their family and other healthcare professionals.
The focus is on evaluating how home nursing can support and improve the health and well-being of the individual and their family.
Home nursing is for individuals who require healthcare services in their own homes. It involves individualized nursing based on an assessment of each person’s needs. A nurse or occupational therapist plans the service in collaboration with the individual and their caregivers.
Home nursing does not provide continuous presence or supervision in the home.
Home nursing is provided free of charge.
The need for home nursing services is continuously reassessed.
When an individual no longer requires home nursing, discharge planning begins in consultation with the individual and their caregivers. In some cases, the person has regained independence or requires other services beyond home nursing.
Discharge from home nursing is carried out in collaboration with the individual, their caregivers, and other relevant institutions.
The client’s home is the workplace for our staff. Good working conditions and safety must be ensured.
The client or their representative must agree that the home environment meets these requirements and sign a consent form.
If conditions in the home pose a risk to the safety of the client or Home Nursing staff, such as disorder or violence, appropriate measures are taken immediately.
Nursing services may need to be postponed while guidance and support are sought.
The goal of Home Nursing is to provide individuals with appropriate support to increase their independence and enable them to remain at home in the most normal and optimal conditions possible, given their health status and social circumstances.
The role of Home Nursing is to deliver individualized nursing that emphasizes prevention, health maintenance, health promotion, professional expertise, and collaboration.
