The signed memorandum of understanding emphasises cooperation between systems, a comprehensive approach and continuity in the service of rehabilitation. Individuals in rehabilitation should be followed systematically between service systems if they need service from more than one system and it will be clear how responsibility for the service is transferred between systems.
