For parties subject to mandatory reporting

  • The purpose of mandatory reporting is to analyse the data with the aim of improving aviation safety, preventing accidents and learning from experience.

  • The Icelandic Transportation Agency does not provide information on individual incidents to the public.

Serious Incidents and Accidents and  Serious Air Traffic Incidents  shall be reported immediately to the Icelandic Transportation Safety Board - 24/7 standby duty mobile number: (+354) 660 0336.

Mandatory Occurrence Reporting

In Iceland, you are required to file a report to the Icelandic Civil Aviation Authority about any occurrence you are involved with. The requirement to report is personal and mandatory.

Mandatory Occurrence Reporting (MORs) is to facilitate collection and exchange of information on actual or potential safety hazards and deficiencies and contribute to the prevention of aircraft accidents.

Mandatory occurrence is any occurrence which may represent a significant risk to aviation safety and which fall into the following categories:

  • Occurrences related to the operation of the aircraft.

  • Occurrences related to technical conditions, maintenance and repair of aircraft.

  • Occurrences related to air navigation services and facilities.

  • Occurrences related to aerodromes and ground services.

All mandatory occurrence reports shall be sent through the ECCAIRS database.

Voluntary Occurrence Reporting

Voluntary Occurrence Reports (VORs) should be reported in the same format as MORs, all reports are triaged and prioritized individually, processed and analyzed together.

Voluntary reports are classed as;

  • Occurrences not captured by the mandatory reporting system

  • Other safety related information which is perceived by the reporter as an actual or potential hazard to aviation safety

Voluntary reporting moves us from a reactive process towards proactive process, helping us identify safety concerns and allowing safety improvement measures to be implemented before they escalate.

All voluntary occurrence reports shall be sent through the ECCAIRS database.

Confidentiality

The person who reports an aviation incident will not be punished nor will he be penalized even if it is a violation

  • of the provisions of the law is related to the incident

  • of the rules established on the basis of that law

However, this does not apply if an incident occurs due to

  • intention

  • gross negligence

  • the influence of alcohol, stimulants or anesthetics.

All employees who receive occurrence reports are bound by confidentiality about the content of the reports. Information about individual aviation incidents may not be released to the public.

The process

  • All occurrence reports are entered in an international database, ECCAIRS.

  • The incidents are then examined and a decision is made as to whether more information is needed.

The ECCAIRS database, which is sponsored and produced by the Council of Europe, is designed specifically to keep records of aviation incidents and is used by civil aviation authorities, investigative bodies and air carriers of their choice.

  • Regulation 900/2017 (EU 376/2014) is intended to ensure the notification, collection, storage, protection, exchange, dissemination and analysis of relevant safety information in aviation.

  • Objectives of Acts and Regulations on Incidents

    • Security action can be taken based on information analysis

    • Access to security information will be maintained with confidentiality and increased protection for reporters

    • Report incidents without apportioning blame or liability if accidents and serious incidents can be prevented.
