Each consultation emphasises individualised and professional care. The clinic provides targeted counselling, education, and support with the aim of improving health, preventing complications, and helping individuals manage illness. The consultations are intended for patients with chronic health conditions who are under follow-up after treatment, diagnosis, or hospitalisation, and/or who require specialised education and counselling on treatment, medication, lifestyle changes, and other interventions.
