To create a request form for the web you can exploit the Form content type.

You can embed a Form into the description field on Organization subpage for the request form to appear on the website

The Recipient field indicates which e-mail address the answers to the form are sent to

Then you can add more fields still default fields (name and email) by creating the Form Fields

It is a good idea to use dropdown type and then write in options the options to be displayed

The Email config field on Form Field can then be used to determine different addresses to which a particular field is connected.

To enable email config on Form Field, select which form field determines the email address to which it is sent (this is the Recipient Form Field Decider on Form type

You can then have different addresses depending on the user's choice
