When is a case referred to the coordination team?
If it is unclear what an individual's next steps are with their service provider. Reasons may include that treatment with the service provider has been fully exhausted, a service has been denied, it is unclear which service provider can assist with rehabilitation, an individual has been denied disability because their rehabilitation is not fully exhausted, or if no solution is available.
