What are coordination teams?

The coordination teams are a collaborative platform for service providers in the field of rehabilitation. They are used to increase cooperation between service providers, where information can be shared in meetings.

The purpose of coordination teams is to promote continuity in services and subsistence, help ensure that individuals receive the right service at the right time, and promote progress and a holistic approach to services based on the needs of each individual. The teams also aim to ensure continuous rehabilitation for those who need services from more than one provider or move between service providers.
