A Letter of Good Standing from a competent authority confirming that your license to practice has not been suspended or revoked and that the license is valid and unrestricted. The Letter of Good Standing has to have been issued less than three months before being received by The Directorate of Health.
A certificate from a competent authority which states the level of the applicant's education under Article 11 of Directive 2005/36/EC. This only applies if the profession in question is registered in the country of study. Information about competent authorities.
Detailed information about your studies. List of courses that the applicant has completed with information on units, duration, number of clinical sessions, competence criteria and detailed course descriptions. It must be confirmed that the course descriptions apply to the applicant and that they apply to the years that the applicant studied. For example, if you studied in 2019-2024, the course descriptions must apply to those years. Note that the office does not retrieve data through links.
A signed certificate of professional competence. With information on the job title, the main tasks and the job duration. Optional but could be useful.
