Application process

  1. Notify your employer using this form. It is recommended that you notify your employer as soon as possible about the use of the grief leave.

  2. Send a copy of the notification. It can be done electronically.

  3. Apply.

  4. A medical certificate or confirmation from a hospital stating the duration of the pregnancy and the parents of the child.

  5. If the parent has submitted an application before the time of the miscarriage or stillbirth, a new notification may be required for the grief leave procedure.

One notification for each employer

If the parent was with more than 1 employer during the 6 months period before the loss of the child, one notification must be completed for each employer.

A retirement certificate if the parent has retired from the workplace.
