Permission to become a daycare parent in a private home

Necessary supporting documents with renewal of application:

• Consent for the procurement of criminal records of all members of the household over the age of 15

• Medical certificate - (confirming the health of the person in question and whether the applicant has chronic illnesses, physical or mental illnesses that affect his/her daily activity)

  • A medical certificate may be obtained from a general practitioner.
