Operating licences for support and assistance to persons with addiction
Social service committees shall encourage individuals with addiction problems who have received treatment and medical assistance, to receive the necessary support and assistance to live a normal and healthy life after treatment.
An application for a licence to operate such a service must be submitted.
Supporting documents with application
A criminal record of the applicant or the representative of the company.
CV of the applicant or company head
If the applicant is a company, the following documents shall also be included:
Operational plan
Annual financial statement
Registration of a company in the Register of Taxes on Undertakings, stating the legal form, composition of the board of directors and the purpose of the company.
