People with disabilities have the right to housing in accordance with their needs and wishes and the right to choose
place of residence
where and with whom it lives, on an equal basis with others.
Disabled people also have the right to social services that enable them to live in their own home and that contribute to their full integration and participation in society. Services for persons with disabilities may not be made conditional on their living in a particular form of residence.

Last annual financial statement (if the company is already in business)
Registration of a company in the Register of Taxes on the Company, stating the legal form, composition of the Board of Directors and the purpose of the Company
Information on the premises where the service is provided. Location, size, real estate number according to the register of real estate and residential number if applicable.
